Our Outreach program equips and provides seniors with information, referrals and advocacy regarding available community services, resources and health care options. We visit and telephone identified lonely, vulnerable, isolated, self-neglecting elders to help meet their needs and keep them safe in their homes. We organize and assist with reading, comprehension, and completion of forms, mail and other paperwork. We resolve and assist with medical appointments, money management and representative payee services.
